Our Services

Daily Health Monitoring

What if my loved one forgets things easily or refuses medications?

A: We use memory-friendly approaches to support routines and medication needs, while respecting the client's choices and care plan. Any concerns about missed or refused medications are communicated appropriately.

Can you send updates after each visit?

A: Absolutely. We provide a brief update after each visit through your preferred method.

Our nurses provide daily visits or scheduled check-ins to monitor vital signs, ensure medications are taken on time, and keep your loved one’s health on track. We notice the small changes—before they become big issues.

What’s Included:

Blood pressure, heart rate & oxygen monitoring

Medication support, reminders & administration as authorized

Personalized care updates for families

Post-Hospital Recovery

How soon after discharge can a nurse arrive?

A: We do our best to arrange support promptly following discharge, subject to availability. We recommend contacting us as early as possible to coordinate your care.

Will you coordinate with my doctor?

A: Yes. With your consent, we can communicate with your physician, discharge planner, or other members of your healthcare team as appropriate to support continuity of care.

What if my loved one becomes distressed, agitated, or disoriented?

A: Our team uses calm, respectful approaches such as reassurance, redirection, and reducing triggers to help support your loved one during moments of distress or confusion. We also communicate significant changes or concerns to the family or appropriate healthcare professional when needed.
